Prescription Drop Off

This form is to request the processing of new prescriptions only. For Refills, please call (800) 628-7427.

Please note:

Sunday and Federal Holiday requests will be processed on the next business day (Main Pharmacy: ScriptCenter® pickup available 24/7; Satellite Pharmacy: ScriptCenter® pickup Monday - Friday: 8 a.m. - 4 p.m., and Drive-Thru pickup Monday - Friday: 8 a.m. - 4 p.m.).
Forms must be submitted by 4 p.m. to receive prescription by the next business day. All submissions after 4 p.m. will be available by 48 business hours.
Paper prescriptions will need to be physically brought into the pharmacy to be processed.
Refrigerated prescriptions cannot be dispensed at the ScriptCenter®
Do not input any Protected Health Information (PHI)

Prescriptions will be available for pick-up 24 business hours at the location you select, if submitted by 4 p.m. Forms submitted after 4 p.m. will be available for pick-up 48 business hours.
