The Defense Health Agency has been hard at work transforming the way patients access care and providers deliver care. NMCP is offering patients a new way to manage and monitor their health in between medical visits – My Military Health Care Companion.
Care Companion’s secure, automated chats are a new way for patients to manage a chronic condition like type 2 diabetes or hypertension or ask questions about lifestyle management without making an appointment with their care team. The new platform allows patients to get answers when it’s convenient for them in a safe, secure, and easy to use environment. It’s the perfect option for anyone who wants to engage with their health care on their terms.
Patients will be automatically enrolled into Care Companion if they meet specific health criteria. Patients will receive an email from
CareCompanion@health.mil notifying them of their enrollment into one or more of four programs (
Type 2 Diabetes, Hypertension, Lifestyle & Wellness, and coming soon
Postpartum Care) and a link to access their care. Patients must have email address associated with their Genesis record to receive a notice.
