Wednesday: 12:30 - 3:30 p.m. COVID ONLY​
Thursday: 9-11:30 a.m., 12:30-4 p.m. (checking in the last patient at 11 a.m. and 3:30 p.m.)
Closed 11:30 a..m-12:30 p.m., Monday thru Friday for lunch

Ensure your immunizations records are up-to-date by sending copies to our Immunizations Clinic via your MHS GENESIS Patient Portal. Send a secure message with the subject titled: "NH Jacksonville Immunizations Clinic" and attach a copy of your:

- DH680 (Florida) shot form for school
- Overseas Screening (dependents)
- Travel Screening
- Immunization Record Updated
